ANNISTON, Ala. - Memorial services for Mr. Edward Polaczyk, 55, of Anniston, were held at Chapel Hill Funeral Home in Anniston, Ala., Friday, July 10, at 11 a.m. with Fr. Bryan Lowe and Rev. James Macey officiating.

Mr. Polaczyk passed away Monday, July 6, 2009, at NEARMC in Anniston.

Mr. Polaczyk is survived by his mother, Susan Henderson; sisters, Sheri Menegaz, Patti Westerholm, Joyce Makowsky, Kathy Milkiewicz; brothers, Harold Polaczyk, John Polaczyk, Leo Polaczyk; brothers-in-law, Denis Menegaz, Todd Milkiewicz; sisters-in-law, Shelly Polaczyk, Mary Polaczyk; nieces and nephews, Katie Menegaz, Peter Menegaz, Carla Menegaz, Amy Menegaz, Jessica Diedrick, Amanda Polaczyk, April Milkiewicz, Sarah Milkiewicz, Leah Baker, Lauren Poe, Logan Poe, Brooke Milkiewicz, Kevin Polaczyk, Corey Polaczyk, Nicky Corbett; second nieces and nephews, Isabella Menegaz, Levi McCarty, Landen Hegward, Keylyn Diedrick, Alexandra Diedrick, and Caleb Zimmerman; many aunts, uncles and cousins.

Mr. Polaczyk was preceded in death by his father, Edward Polaczyk; stepmother, Beatrice Polaczyk; brother, Richard Polaczyk.

Mr. Polaczyk was a native of Michigan and resident of Anniston. He was employed as a truck driver.
